Uploading your documents

To drive using the Driver app, you are required to upload certain documents to get your account set up. Refer to the information below when uploading each document.

If you have a copy of a previous driver’s license

If you still have your previous driver’s license (or a picture/photocopy), all you need to do is provide a photo of that driver’s license to Checkr. Click on Upload Required Documents to submit your previous driver’s license.

Tips for uploading your documents

Following these suggestions and rules when uploading your documents can help the process go smoother.

Make sure all required details are clearly visible

Ensure that your document contains all required details before uploading. The entire document should be clearly visible.

Upload your driver's license first

This will speed up the approval process when we need to cross-check your license with other documents.

Upload one image or PDF at a time

You can go here to get more information about uploading your documents through the Driver app.

Submit original documents only

We do not accept photocopied documents, so please make sure you only upload originals.

Note

It may take up to 48 hours for your documents to be reviewed and verified.

If you do not have your previous license

  • Come into our Greenlight Hub during office hours and an Uber Expert can provide you with the form, envelope, stamp, and assistance in filling out the form. Just bring a $5 check made out to the Ohio Treasurer of State with you to your visit.

    Once you receive your driver record, submit an electronic copy of it here.

    • Download and print Form 1173 to request a previous driver’s license application from the Ohio BMV. See our sample form for guidance in filling this out.

    • Download and print our note template, which specifically requests a previous application.

      • Fill in the number of years needed and sign
      • If you are under 23, Uber requires 3 years of prior driving experience. Those people age 23 and over are required to prove at least one year.
    • Make a $5 check or money order payable to Ohio Treasurer of State

    • Mail the above 3 items to the following address:
      Ohio Bureau of Motor Vehicles
      Attn: BMV Records
      P.O. Box 16520
      Columbus, Ohio 43216-6520

    • Once you receive your results, upload to the Checkr portal.
