Trade dress
Kentucky state law REQUIRES Uber drivers to display an Uber decal in your vehicle when picking up passengers and while taking trips arranged through the Uber app. The decal must be affixed to your front windshield on the passenger side during these times. It may be removed when you are not logged in to the Uber app. You can have an Uber decal mailed to you by filling out this form or print your own for temporary use by using the link below.

Kentucky state law requires Uber drivers to have their vehicle inspected by a third party mechanic within 30 days of being activated to receive ride requests by a TNC. This vehicle inspection must be done on an annual basis. Before going to a vehicle inspection location, please download and print the form below.
