Beginning January 1, 2019: TLC Driver License applicants must take the TLC Driver License exam at a PSI location. TLC Driver License Applicants should receive an email from PSI Services after completing the TLC Driver Education course. The email will provide instructions on how to make an appointment to take the TLC Driver License Exam. For more information, please visit

TLC medical exam

The TLC requires applicants to take a quick, non-intrusive medical exam. We recommend you do this on one of your visits to the Greenlight Hub.

Step 1

Schedule your medical exam

Option 1: During a course visit

Save yourself the extra trip and take the medical exam during your Defensive Driving or WAV Course visit to the Uber office. No extra appointment is necessary.

Option 2: Visit the Queens Greenlight Hub

If you can't complete the medical exam during your course visit, come to the Queens Greenlight Hub to get a free medical exam by Pager Medical, our official medical exam partner.

Option 3: Go to a doctor of your choice

Print the TLC medical form and bring to any certified physician to complete.

Step 2

Upload your medical form to the TLC's website

You can submit your completed medical form to the TLC through their online TLC Upload Portal. Please have the following information ready before you begin:

  • Your current TLC issued application number
  • The zip code of your mailing address
  • The last 5 digits of your Social Security Number (SSN) or Employer Identification Number (EIN)
