7: In "Remarque" note place and date, sign the document

8: Upload all pages on the Uber Driver App

9: Send the filled out form to the Service de la consommation et des affaires vétérinaires (SCAV), rue Jehanne-de-Hochberg 5, Case postale 1, 2002 Neuchâtel 2 or via e-mail to scav@ne.ch
