3: Note your your own private address, phone number and e-mail address

Verantwortliche Person

4: Note your first and last name, your date of birth, your place of origin and nationality

5: Note your phone number and your e-mail

Betriebsdaten

6: In "Betriebsmerkmale" select "Einzelbetrieb"

7: In "Kurze Umschreibung der Tätigkeit" note "Kurierdienst: Transport von verpackten Waren"

7: In "Besitzen Sie Zertifizierungen?" select "Nein"

8: In "Datum Betriebsaufnahme" note the current date

9: Select "Ich stimme der Datenbearbeitung zu" and click on Senden

The document was sent to the Kantonale Laboratorium, Muesmattstrasse 19, 3012 Bern

11: Take a screenshot of the e-mail confirmation from noreply@forms.be.ch and upload it to the Uber Driver App

2: Select "Neuerfassung"

3: In "Betriebsdaten" note your first and last name and your phone number

4: In "Betriebsadresse" note your own private address, phone number and e-mail address

5: In "Betriebsmerkmale" select "Einzelbetrieb"

6: In "Charakterisierung des Betriebs" note "Kurierdienst: Transport von verpackten Waren"

7: In "Weitere Angaben" note the current date

8: In "Bestätigung der Vollständigkeit" note your first and last name, note place and date. Sign the document

9: Upload all pages on the Uber Driver App

10: Send the filled out Meldeformular to the Kantonale Laboratorium, Muesmattstrasse 19, 3012 Bern or via e-mail to Info.kl@be.ch
