3: In "Betriebsadresse" note your own private address and your phone number

4: In "Betriebsart" note Andere "Kurierdienst, Transport von verpackten Waren"

5: Note your first and last name on the right side next to "bisher"

6: Note on the left side your own private address, phone number and e-mail address

7: Note place and date, sign the document

8: Upload all pages on the Uber Driver App

9: Send the filled out form to the Amt für Lebensmittelsicherheit und Veterinärwesen Basel-Landschaft, Gräubernstrasse 12, 4410 Liesthal or via e-mail to alv@bl.ch
