1. Apri il modulo in alto "Meldeformular für Lebensmittelbetriebe, sowie Betriebe die Tätowierungen oder Permanent-Make-up anbieten"

2. Seleziona "Neuerfassung"

3. In "Betriebsadresse" e "Verantwortliche Person", indica il tuo nome e cognome, il tuo indirizzo privato, il numero di telefono e l'indirizzo email

4. In "Betriebsart", specifica Andere: "Kurierdienst, Transport von verpackten Waren"

5. Indica il luogo e la data e infine firma il documento

6. Carica tutte le pagine sull'app Uber Driver

7. Invia a mezzo posta il Meldeformular compilato a Gesundheitsdepartement Basel-Stadt, Kannenfeldstrasse 2, 4056 Basel o tramite email all'indirizzo sekr.kantonslabor@bs.ch
