If no copy of a previous driver's license
You'll have to obtain a driver record
Download and print Form 1173 to request a previous Driver’s License Application from the Ohio BMV. See our sample form for guidance in filling this out.
Download and print our note template, which specifically requests a “Previous” application.
- Fill in the number of years needed and sign.
- If you are under 23, Uber requires three years of prior driving experience. Partners 23 and over are required to prove at least one year.
Make a $5.00 check or money order payable to Ohio Treasurer of State.
Mail the above three items to the following address:
