Driver accreditation with the CPVV
Once you have completed your online application for a driver accreditation (DA), the next step is to complete a medical assessment with a doctor. You will need to print and take the following medical assessment form to your appointment.
After your medical appointment, you will need to upload your completed medical assessment to the Commercial Passenger Vehicles Victoria (CPVV) document portal, using credentials provided to you by CPVV after you have submitted your online application.
- If applicable, please bring your eyeglasses and medical prescriptions to the medical appointment.
- Failure to bring these, may delay your process and require you to rebook another medical assessment to obtain your driver accreditation.
- If you have received an email from CPVV stating you must provide certified documents, please also bring these to your medical appointment. Your GP will be able to certify these for you.
Discounted medical assessments
Jobfit offers a discounted medical assessment of $77 (incl. GST), or $73 on weekends to Uber driver-partners. The discount will be offered at the time of booking.
