Proving driving history
What you need to know
If you signed up to drive with Uber and recently had a new license issued, you will need to indicate that you have had a US license for at least one year (three years if you are 21 or 22) to successfully complete your background check.
For licenses issued in Connecticut
If you no longer have a photocopy of your previous license, please complete these two steps: Submit a Copy Records Request (form J-23) for Driver License File Information:
- Download the form here and mail it to Department of Motor Vehicles, Copy Records Unit, 60 State Street, Wethersfield, CT 06161 with a $20 check enclosed or deliver it to a DMV location near you.
- Upload a clear photo or scan of the MVR after entering all required information
For licenses not issued in Connecticut
If you don't have a copy of your previous license, you can do the following two steps:
- Visit the DMV in the state that your previous license was issued in and ask for a copy of your drivers licensing history with the original issue date listed. Make sure the original issue date is at least one year in the past
- Upload or scan a clear photo or scan of the letter after entering all required information here.

Visit the Applicant Portal
If you are unable to obtain the items above, reach out to your previous state’s Department of Motor Vehicles and request a copy of your motorized vehicle record (MVR) showing at least one year of previous driving history. Once you have obtained a copy of your 10-Year MVR report, please visit the Applicant Portal and follow the instructions to obtain prove of your driving history.
